Grandparent

Adult Brother or Sister
Guardian Having Legal Custody or Control
of a Minor Child
Attorney Representing the Registrant
Adoption Agencies and Private Attorneys
Persons or Agencies Empowered by Statute
or Appointed by a Court to Act on the Registrant's Behalf
